Three leading care providers will be working in partnership to deliver home care services across the London Borough of Waltham Forest.
The Waltham Forest Outcome Based Home Care Service, which launches on 20 April, will be a lifeline to over 800 people with learning or physical disabilities, mental health conditions, children and older people living in their own homes.
Dedicated staff will work with clients to help them meet their goals and achieve greater independence. This could be anything from learning a new skill, to setting up a direct debit or providing respite for families who have disabled children.
The outcomes-based partnership, which consists of Allied Healthcare, Housing 21 and Leonard Cheshire Disability, will be working in conjunction with the London Borough of Waltham Forest and the local Primary Care Trust.
Commenting on Housing 21’s successful bid, Les Clarke, Director of Older People’s Services said: “We are delighted to be a member of The Outcomes Partnership and look forward to delivering care services in the London Borough of Waltham Forest.”
The three organisations have signed up to a Partnership Charter in which they have agreed to work together to ensure that service users’ life choices are provided with flexibility, quality and value for money.
