Qualified Social Worker - Older Persons

Our clients require 4 social workers asap to work in older adults undertaking assessments/setting up care packages and participating on locality duty rota and also assisting Information Officers with Access Point work.

You will manage your own caseload, make community care and carer's assessments, and set up care plans . You will also be working closely with your managers on any adult protection issues that arise, as well as liaising with health professionals to commission services for clients.

In order to be successful for this position, you must have experience in an Older Persons community setting with good knowledge of the Care Management Process. You must also be fully DipSW qualified, GSCC registered and have a recent CRB. In return you will be working in an excellent location; close to all transport links and local amenities as well as being offered competitive rates of pay.
