Project Lead: 0-19 Care Pathway

Our client, and Essex based NHS organisation, is currently seeking a Project Lead: 0-19 Care Pathway. Reporting to the senior management on an interim basis of 3 - 5 months, you will be bringing together public health nursing, health improvement, education, voluntary sector, and local authority services to provide an integrated function for the delivery of a multi-agency team solution for preventative health.
You will be commissioning comprehensive Community Children's Services to deliver Early Intervention and Prevention through public health nursing (Health Visiting Services, School Health Advisory Services), Child Protection Services and Paediatric Medical and Therapy Team Services to the children and young people of Essex and their parents and carers, placing the child at the centre of all decisions and service provision delivered.
This will include responsibility for joint commissioning arrangements for Children's Centre and Extended Schools Services, along with
- Utilising a Health Needs Assessment provided by the commissioning arm to ascertain the health and other needs of children and young people of the locality.
- Designing a project template to bring together services are brought together to focus their work more closely towards meeting identified public health needs and to work as an integrated team with health improvement, voluntary sector and other statutory agencies including education and the local authority.
- Ensuring that the universal maternal support and child development
service is delivered to all families, giving higher levels of targeted support to individuals and communities, particularly in those super output areas highlighted as having the greatest need, whether by disability, serious ill health, disadvantage or other stresses.
- Developing multi-agency teams, incorporating public health nursing , existing members of health improvement teams and relevant partners including primary mental health workers, community mothers, early years workers, children's centres etc
Our client requires that any potential candidate be of a registered Health Visiting, School Nursing or Qualified Public Health Nursing background. You should also be in possession of
- Excellent project management skills, with the ability to work independently on projects of your own creation.
- Good working knowledge of NHS policy and procedure, with an excellent understanding of public health.
- The ability to move between locations, partnerships, stakeholders and agencies quickly and independently.
