Swim are looking for a Care Coorindinator to work as part of a team that is responsible for all aspects of delivering an effective and efficient quality domiciliary care service with significant Local Authority and private contracts.

In this position you will be responsible for the efficient co-ordination of Care Worker visits to Service Users, meeting all National Minimum Care Standards and implementing Anchor Care's quality assurance systems and procedures.


Summary of Duties and Responsibilities:

Operational Management

·Co-ordinate a care package caseload, including the allocation of care worker visits and maintenance of a computerised rostering system. Ensure all paper files for Senior/Care Workers and Service Users are up to date and mirror information held on the computer system and vice-versa.

·Close Liaison with Service Users, their representatives, your team, Senior/Care Workers, Local Authority purchasers and others.

·To highlight and monitor recruitment needs both within the field and the office, to liaise with your Senior Co-ordinator and Monitoring Officers to ensure quick response to needs, to address advertising, interviewing and ultimately induction of new staff. To ensure staff files are up to date.

People Skills

·Communicate in a professional manner with other members of the team, Care Workers, Service Users and their representatives, purchasers and other agencies to promote a positive and professional image of Anchor Care.

·Ensure that complaints and conflict are dealt with positively and effectively and that individuals are communicated to clearly, concisely and in a timely manner.

Business Skills

·Work effectively with the finance teams to ensure efficient income collection.

·Participate in the monitoring of the Branch performance against agreed budgets as outlined in the Business Plan. Provide daily/weekly/monthly information as directed and respond to system driven reports highlighting administrative work required on the computer system and on the paper files, e.g. clear rota's, unapproved hours and unauthorised hours.

·To keep abreast of care packages won and care packages lost – report on this as requested. Be aware of market changes and new opportunities for Anchor Care to explore.
